What You Will Do
Duties and responsibilities include but are not limited to:
- Provide optimal care to each of your assigned patient and ensure that each patient’s needs are met
- You will perform a wide range of nursing care to patients in home setting including assessment medicine administration, wound care, nutrition administration, assist with activities of daily living, etc.

- Administer medication, treatment, and therapy as prescribed
- Produce accurate and properly formatted documentation of your shift with the patient, even under tight time constraints, to effectively provide leverage to our patients
- Communicate effectively and collaborate with the patient’s family and other multi-disciplinary team for the continuity of care of the patient
- Ensure patient safety at all times: infection, falls, aspiration, seizure etc.
- Maintain patient privacy at all times by adhering to the HIPPA law
- Communicate effectively with patients/family members regarding any concerns or questions they might have pertaining their care or that of their loved one
- Promptly communicate with the family, physician, and your clinical manager when there is change in your patient’s health
- Possess the ability to lift, transfer, and maneuver incapacitated patients in the home setting
- Provide care based on New Jersey Standards of Practice and be compliant with Oyster Healthcare policy and regulatory requirements
- Always, maintain a strong ongoing rapport with our patients/families and your fellow colleagues.
Qualifications
- Clean, current New Jersey LPN license
- Current CPR card
- +1 year clinical experience as a LPN
- Previous experience in the home care setting is desired
- Have patience and compassion while caring for patients
- Have excellent verbal/written English communication skills
